As The Value Care Project draws to a close, I had the distinct pleasure of presenting my learnings at the 24th International Conference on Integrated Care (ICIC24). This conference provided a unique platform to share insights and discuss the critical role of digital health within integrated care settings.
In my presentation, I focused on our experiences implementing digital health solutions in ICPOP Kerry/Cork, where we recruited participants for the project. I highlighted the challenges and successes we encountered while striving to integrate digital tools into our care models, aiming to enhance patient outcomes and streamline healthcare processes.
Through this project, we’ve learned the importance of engaging stakeholders across all levels—healthcare providers, patients, and technology partners—to ensure a holistic approach to care. I shared specific strategies we employed to overcome barriers to adoption and foster a culture of collaboration among diverse teams.
The insights gained from the conference were invaluable, providing me with fresh perspectives and innovative ideas to further our efforts in integrated care. As we move forward, I am excited about the potential of digital health to transform how we deliver care in Ireland and improve the overall patient experience.
